I just need someone to listen to me!

I don't think I've ever created a post but I'm at my wits end. I have 2 children. My oldest son is almost 3. He was a reflux baby. I put him on Zantac and it was like magic. A new baby in a matter of minutes. We have a family history of stomach issues. My father has Celiac Disease. My mother and I suffer from acid reflux. My grandfather passed away from esophageal cancer caused by untreated acid reflux. My mother in law required surgery to fix her stomach because she couldn't keep food down. This ain't my first rodeo.

Enter my second child. He started having issues at 2 weeks old. Spitting up a lot, choking on acid, coughing. We put him on zantac. It made no difference. He was miserable all the time. We went to the hospital to get an ultrasound to be checked for pyloric stenosis. It was fine. His doctor determined that he was just a happy spitter. Except he wasn't happy. He was in pain. He wasn't gaining weight. Still hasn't made it out of the third percentile and he's almost 4 months old. I was disappointed that my pediatrician wasn't listening, knowing my family history. We've been going there for almost 3 years but I decided to find another doctor.

In the meantime, he's still projectile vomiting regularly. Miserable. Not sleeping. Not gaining weight. I took him to urgent care at the children's hospital. They did an x-ray. Nothing detected. Diagnosis was colic. I started a probiotic. He got a little better before getting a whole lot worse. I made an appointment with a new doctor. He still wasn't gaining weight. Still vomiting regularly. She did start him on a different medicine. It seemed to help a little at first.

Fast forward to yesterday. He was napping and had been having a rough stomach day, so I went to check on him. He was asleep in a puddle of vomit. I managed to clean him up while still asleep. This morning, he spit up a good bit, but I assumed he got too full. He went back to sleep afterwards. I went back to get him about an hour later because I heard him awake on the monitor. He was working on a poop, so I let him finish. I changed him and put new clothes on him. Then he vomited again. It was like a fountain. Covered from head to toe (not the first time- his carseat is disgusting from this). It had been over an hour since he had eaten.

This isn't normal. Nobody seems concerned. If it's normal- why isn't he gaining weight? Why does he have forceful, awful smelling gas all day? Why does he vomit all the time? Why does he only poop once a week? I'm at a loss and don't know where to go from here. I made another appointment with his doctor. Does he have to end up in the hospital before someone will listen? If you've read this far, thank you. I just wanted someone to listen.

UPDATE: Evidently my scale is wrong. He's still not 12 pounds. He's only 11 pounds, 6 ounces. She said he's only gaining about a third of an ounce a day. This places him in the first percentile. We did a transfer test to see how much milk he was getting in a feed. He took in 3 ounces. He spit up quite a bit while eating, so I'm estimating it was closer to 4. She suggested giving a bottle after he nurses. We go back in next week for a weight check.
