Postpartum bleeding! Advice! tmi!

I'm 2 weeks postpartum as of yesterday[monday], bleeding had stopped for a few days, then today I woke up from a nap and pad was soaked. I wiped myself and each time clots were on tissue like below pictures... I cleaned myself and it felt like I had to pee again so I sat down, but nothing came out..instead when I wiped again TOP picture was what happened when I wiped. In the 2 weeks blood has went from red/pink/light to dark and clot... should I be concerned?
