I would say no, unless you have some specific questions, concerns, or a medical history that would require a doctors appointment.

No, but I did one. I wanted to make sure after 11 years on the pill I didn’t have any major issues. They did a ton of tests and ultrasounds and cleared us. Unfortunately she missed that I had multiple issues that weren’t found until I went to a specialist 3 years later. 
It’s always good to get baseline bloodwork done though and make sure you are in good health before trying to conceive.

Not necessary since many people don’t have one, but nice to do. I was tested for my MMR immunity (I think that is the one) and then discussed my cycles to make sure I was ovulating when I should, got a prescription for prenatals, got a pap to make sure everything was normal, etc.
