High BP readings on at home monitor

I have no idea what I should do about these numbers. When I have my BP measured by a nurse twice a week they are ok. But at home they can get a bit high and the readings are always different, even after retesting right after. They said I should go to the hospital if it’s high since I’m high risk but I don’t know. Yesterday my BP was 115/76 with a nurse. Today 123/79.
