Protocol for getting trigger shot?

So my cycles lately have been weird with spotting begging at about 10 DPO. I’m thinking low progesterone. This last cycle, which was only 19 days long I’m pretty sure I didn’t ovulate at all. I go in next Tuesday which should be close to my ovulation day if I ever stop bleeding.
My progesterone was 0.04 on cd 3 (which was supposed to be cd 21). I’m hoping they do an ultrasound to check follicles. Just not sure what to expect. 
How did you start getting your trigger shot? Or other meds? What’s the protocol on all of that?

Normal couples who just start trying are told to wait a year before seeing a specialist. I think over 35 you only have to wait 6 months. But for us we tried a year with nothing I found a specialist and we went from there. I have pcos so we tried 6/7 rounds of clomid which never worked. Then 4/5 rounds of letrozole which on the 6th round we did metformin and the highest dose of letrozole and I made an egg. First one in five years. I triggered then had an iui. My husband also had a low count, so iui was suggested, but you can do timed intercourse. Our second iui took and I have a two year old. We are currently on our second iui cycle trying for baby two. They ate probably going to try everything else before moving you straight to trigger. And check with your insurance the trigger can be expensive and might require other steps first.
