VERY itchy/painful nipples 😭

I am going crazy with these constantly itchy, burning, sore nipples! I’m between 13-14 weeks now. For my t-shirt to slightly touch them makes me cringe. Does anyone have any tips for relief? Do I need to already be looking into some kind of nipple cream?
