What causes the pill to fail besides human error?

Hi! I’m new to the pill and heard people have gotten pregnant on the pill even though they took it perfectly without missing a pill, taking another antibiotic and more so I was wondering, how can someone get pregnant on the pill if they take all the correct steps?

The pill is 99.9% when used perfectly but most dont use it perfectly so it drops to 91%. Take it on time, if you vomit or have diarrhea within 3 hours of taking it its a missed pill use condoms for 7 days, make sure you don't have other meds that interact with it- hiv meds, high blood pressure meds, antibiotics, antiseizure meds- also used for migraines and bipolar disorder, and st John's wort. So not just about taking it on time. Thats why iuds and implants are 20X more effective even tho they are also 99.9% effective because they have little to no room for user error. User error accounts for 95% of pregnancies on birth control. 
I used mini pills for 2 years without condoms or pull out and never got pregnant perfect use is 99.5% effective as good as female sterilization typical use is 91%. 
I used just condoms for 3 years without pull out. Perfect use is 97% effective typical is 88%. 
I'm on the second year of mirena no condoms or pull out and havent gotten pregnant. 
Even when ppl say they've taken it perfectly i would guarantee 99% of those people are missing something about how they took it wrong. Nothing is 100% effective so it does happen but rarely due to the medication not working.

In my case, I took it perfectly and got pregnant. No birth control is 100 percent effective even with perfect use. Sometimes your body reacts differently to different medications! I was never given a solid reason why mine didn't work at that time (after months of taking it and it working fine).
