I had something similar on my scan and it’s actually the clitoris forming. I know I’m having a girl as I did the genetic blood test and the doctor at the 12 scan confirmed it is a girl by showing me this part in the scan and explaining this is the clitoris forming.

I would guess umbilical cord. They are teeny tiny even at the 20week ultrasound.
