Yes it looks like it could be herpes. Valcyclovir is an antiviral medication used for cold sores and herpes to help the outbreak not last as long. For cold sores its taken as needed when you get a cold sore. Ive heard when you have genital herpes its taken daily to prevent outbreaks. Having hemorrhoids or a fissure doesn't cause herpes. Herpes like other stds can lay dormant for years until something triggers it to cause an outbreak but its more likely to get the first outbreak within a few weeks to a month of when you first came i to contact with it. I would ask the person your sleeping with to get tested as well.

It looks like herpes lesions. Are the anti-virals helping? 
I’ve done a lot of reading on this, and it is totally possible to be with someone for many years and either not become infected, or get infected after quite some time. The issue is that many people (an estimated 80% of the population) have some form of HSV, but many people never develop lesions...however, shedding occurs 10% of the year. This means that you could have acquired the virus either through sex or oral sex while your partner was shedding, without any visible lesions. If your immune system happens to be lower at the time (due to social distancing, pregnancy, your period, etc.), you could become infected. No need to assume your partner is/has cheated.
