How to politely ask someone to get their teeth cleaned?

The guy smokes and has bad teeth. I like him otherwise, but he needs to get his teeth cleaned. How do I tell him this?

Like cleaned at a dentist? Maybe try to make it a fun self care date. Go and get a pedicure the two of you and maybe a new outfit, etc. and then say “you know what makes me feel so relaxed? Getting my teeth clean” and then go 😂

Maybe you could ask him where he goes to the dentist because you were thinking about switching offices. Then when he was like I don’t have a new just be like oh my god you’re supposed to do your teeth cleaned every six months you should go get your teeth cleaned!

My husband usually vapes (he's trying to quit) but when he was out he did take cigarettes from his mom. I just straight up told him "oh you smell like cigarettes, I don't like it" I made him wash his hands, face, beard and brush his teeth. 
If its that his teeth aren't white, maybe buy him whitening tooth paste to try? And then take others advice on getting him to the dentist
