Did I ovulate twice?

Answer

You cant ovulate more than once in a cycle, but you can release multiple eggs during one ovulation period of 12-24 hours. Once that time frame is over, you won’t ovulate again until the next cycle. 
Opks and LH tests only look for a specific level of the lutenizing hormone which peaks prior to ovulation. However, you can have multiple positive tests and not ovulate at all, because that LH mini-peak wasn’t enough to ovulate but enough to show positive on a test. 
Additionally, these tests aren’t typically recommended for women with PCOS and other hormonal imbalances because they’re less reliable for women with these conditions.

It looks like only once, on 11/28? I found that ovulation tests never worked for me and I think a lot of other ladies with PCOS find the same, so I’m not sure how accurate they are for us.
