Help!!!

So i’m going to the doctors today to get tested for a UTI. I am sexually active since the beginning of october. My partner and i were both virgins. I’m 16 and i want to tell my doctor i’m sexually active so they can also test me for any STDs but i’m scared and i’d have to tell the nurse while she takes my height that way my mom wouldn’t hear. Does the doctor have to tell my mom??
