The 3D ultrasounds I know of are elective and not covered by insurance. If it’s something your office offers and they accept your insurance then you’ll likely just pay a small part or co-pay.

Last pregnancy, the hospital did it as part of my regular ultrasound and again as part of a biophysical profile so it was all covered. They consider it part of a diagnostic procedure at the hospital here though.
