12M old waking up to feed! Help!

Hi ladies,

I am exhausted! I really need some advice on what I can do. Here's my situation:

I have an 11 M old (1 yr in a week) we did sleep training around 5 months and it helped a great deal! She is able to fall asleep on her own for naps and bedtime, by just placing her in the crib and we walk away. Its great! We sometimes get some cries but it's just her whining, and she falls asleep within a minute or two.

My issue is that she still wakes up around two times during the night. And the past week or so it has been earlier and earlier each time. Here is an example:

Bedtime... we start a routine at around 520, we feed her her last bottle, bath (if its bath day), change diaper,, change into PJs, brush teeth, read a book or two, say goodnight, and place her down. She'll fall asleep usually right away, between 535-545 shes out for the night on any given night.

Now, the issue comes when she wakes up in the night. She was waking up at around 2am-3am. And then again at 630-700am. These past few weeks it's been anytime from 11am-1am. And then again at 5am and then up for the day at 615ish. The two wakings at night I feed her a bottle. I am aware that I have to wean her off, and I need advice with that. In the past, I have tried to just let her fall back asleep, but she doesn't. She'll start standing and crawling in her crib, whining, banging the crib with her hands and feet, and it becomes very loud and I eventually give in after maybe 45 min to an hour. I go in and she seems super hungry so I feed her (im aware that I have conditioned her this way..) but if I don't go in she won't go back to sleep. Oh also, shes eating full (or semi full) meals during the day, breakfast, lunch, dinner, with some snacks here and there.

I really need help! I'm so exhausted,its been over a year since I got a full night's sleep (thank you pregnancy insomnia). I just don't know how to wean her off and perhaps re-sleep train her.
