High fever🚨

Little Andy has a temperature of 103.7! he can’t sleep, he’s just whining and shaking/shivering. I sat him up to give him a drink and he can’t sit up, he’s just shaking. I just gave him motrin a few mins ago and put one of those patch things on him but what more can I do that’ll help? It’s about to be 5AM, he’s been like this since around 1AM, with a fever this high should I wait for meds to kick in or straight to ER?? Help
