Knife Like Nipples

Ok, is there anything that can be done when your nipples feel like a knife? I know there’s no heating pads on the belly, but can I have it higher up on my breasts? This is my first pregnancy and this is an area I’m actually not clear on. I know I can use it on my back for my sciatica.
