So this is literally between my pelvic area and stomach and it’s painful. What could this be?

Looks like a yEast rash may need an anti fungal cream. But I would go get checked out just in case we are not correct, your doctor will prescribe some thing

It looks like heat rash or chaving... Try some moisturizer without fragrance (baby products often good for that)...if your stomach is chaving on your underwear it could cause this too, especially if they underwear has some metal or plastic ornaments on the front or is a synthetic plastic fiber...best to opt for cotton or silk with no attachments at the front, even the little bows can cause chaving, I always remove them
