Same thing happened to our son at 2 months old and it happened to be a milk protein allergy. But could also be something else in your case not necessarily this.

Go see your doctor for sure but it could be nothing. We had very similar looking diaper at 4.5 months and they said probably just a burst blood vessel from straining too hard. They said come back if it happens a few times in a row or there is other signs of illness like puking or fever or lethargic etc.
