How to make appointment with PCP for mental health?

I want to be officially diagnosed. Can't get to a psychiatrist because the nearest one that accepts my insurance is over an hour away. I have undiagnosed anxiety, depression, and ADHD. I want meds for at least my anxiety. I would like to have medication for my ADHD as well, but as an female adult I think I might have a difficult time being diagnosed with that. I don't care what they give me as long as it helps. I won't lie, I've had 1mg of Xanax before and it really helped(I don't make it a habit of taking medication that isnt prescribed to me). My mental health has really taken a hit in the past year. So how do I even say what the appointment is for when they ask? Do I say mental health? I mean the goal is to get officially diagnosed, but if they would prescribe me something to help I would be so glad. I could finally get some relief from my constant anxiety
