Wedding cake drama

I was reading an advice column today and the woman who wrote is said her one insistence for her wedding was that the groom not shove cake in her face. He agrees beforehand not to. So the big day he grabs her hair and pushed her whole head into the cake and had cupcakes ready so he obviously planned it. She has issues from a car accident and having her face held down in the cake was triggering. So she leaves the wedding and a month later wants a divorce.

The advice writer (Prudie at Slate) says divorce him.

Relationship fine otherwise and he’s apologized.

Is this a red flag or a really stupid mistake.
