Tissue passing at 23rd week.. totally worried

Today after peeing , this chunk fell out .. i am totally worried .. is it miscarriage ? Any body experienced this? I had slight bleeding 7 days before .. went to check up baby was fine. Doctor gave med for same. Today seeing this i am soo scared .. please advice
