Military Pay-Out

So my husband is being medically discharged from the Military with 100% Veterans Benefits. He was given two options for his pay-out. The first is a Severance Check of $460,000 and $4,000 a month for life or the other option is $3500 for life, no Severance Check but our family of 5 will have health insurance for life.

My husband requires a caregiver and can no longer work. He doesn't want a stranger in our home caring for him and without his usual income we won't be able to send our youngest two to daycare. He wants me to be his caregiver but the pay is bleh but I'll get to stay home and care for him and our kids. If I become his caregiver then I'll have health insurance through the company but my 3 kids and husband will still need health care.

What would yall do? Which choice would you make a why?
