I’m scared of losing again

According to the app I ovulated on 6-29, I got a BFP at 12dpo. Went to doctors and they drew blood to test hcg which was 396. So they go to an ultrasound and saw nothing. Does this mean it’s a miscarriage? According to the levels I should be 4 weeks and at 4 weeks you usually see something.
