I measured 20 and was told to use 21… ended up still being too big and use 19. So I would try 17

I also think I have elastic nipples. I bounce between a 17mm and 19mm. I got the MayMom inserts. I find the 19mm more comfortable most of the time to be honest. But if I go by actual sizing, I should be in a 17. I use the 19s with my wearable pump and 17s with my stationary pump. Have you measured at all? I just used a printable measuring mechanism from Spectra. 
Are you getting sore at all? If so, I'd definitely size down. If not, maybe try the 17 for peace of mind.
