What does he mean?

My bf got pretty seriously injured a little more than 2 months ago. He has had a hard time with losing his job and the pain of his injury. He recently started talking again and has communicated that he thinks we should just go back to being friends. He says he "doesn't care" if we cuddle or kiss but the relationship part scares him. I tried to understand what he's scared of but he says he doesn't know how to explain.

Well, maybe he is afraid that (depending on how serious the injury is) he can’t give you the relationship you want. If he doesn’t have a job, he can no longer take care of you financially. If he is paralyzed or no longer mobile, then he can’t care for you physically in the way he used to before. He is probably giving you “an out” or a fee pass to break up here. 
He is maybe now disabled in some way or another and that’s a lot to deal with. Especially in a relationship. He doesn’t want his limitations to apply to you too. Ultimately, you can’t force him to stay in a relationship with you. If he wants to break up, you have to respect that. But make it clear how you feel and what your intentions are. 
Additionally, if it doesn’t work out, don’t feel like it is your fault. The injury was an accident. Serious injuries are hard to handle in a relationship and everyone copes differently. Maybe he needs to break up to find his independence now. If you are okay with it, continue being friends, but don’t cuddle or kiss. Instead, talk and be casual. Be supportive without being his everything. If he wants to break up, you still have to be available to move on.
