It looks like the start of a cold sore, if it gets worse that’s how you’ll know. I would definitely reach out to your doctor.

Is it really tender? Was it accompanied with any tingling or itching prior to the sore? It’s hard to tell, but I don’t think chapped lips usually make a “sore”.
