Should I take to doctor? I just went Monday for 4 month vaccines I told doctor baby had constipation and only said give water and massage belly

No water or juice. It’s normal for babies to not poop everyday. Belly massages only

Is baby in distress, first of all?

Not pooping for 2 days doesn't automatically equate to constipation. Is baby uncomfortable? Hard stomach? Straining? Pushing their legs out? Having hard pellet poops when they do go? If baby seems happy enough, then I wouldn't worry. If baby is clearly uncomfortable, you can do bicycle kicks, tummy massages or try a glycerin suppository. Reserve offering juice (no more than one teaspoon of apple or prune) as a last resort and no water before 6 months due to the risk of water intoxication.
