3 weeks post partum - clot? Placenta? HELP!

I am currently 3 weeks postpartum (my second child) my bleeding had almost stopped and then picked back up along with cramping and clots for a few days today I was cramping really bad and went to the bathroom to change my bad and this fell out in my pad. Does anyone know what it could be??
