can someone help me with making a CPS report?

I’m needing to make a CPS report for two little girls and i haven’t been though this before. I have made a list of signs of abuse/neglect but i didn’t want to call and just ramble on if that makes sense. they’re clearly not being taken care of and their mother is being extremely avoidant since we’ve brought our concerns to her. how does this thing go? will we be anonymous? i mean she will likely know it’s us but at this point i don’t really care. i’m just worried that if this doesn’t go anywhere that we won’t be allowed to see them.
