Early maternity leave

Every doctor n nurse has recommended that I take it easy bc I have a syncope condition (fainting ). Physically this makes it hard for me to return to work so I told my employer I was going to go on maternity leave due to what the doctors suggested. I didn't even think I needed a note bc I've been working there for three yrs so they know it's not that I just don't want to work & why can't my word be enough? If I FEEL like I can't handle sticking out this last month maybe it doesn't matter to me if I think I need my doctors consent. My employer was ok with it at first & now he's asking for a dr note so now I have to ask my new ob if she could write me a note. I meet her for the first time on Jan 2nd (switched doctors) Will she do it?
