I feel like a fool

So, my partner and I were together for 5-ish months and then he broke up with me out of the blue, after a week or two he came back and begged for me back, and so I told him, you need to get help for you mental health but I will take you back while you go through that process. Welp, here we are, 10 months into our relationship and he breaks up with me again, out of the blue, and honestly now that I think about it, In those last 5 months, after I took him back, he still didn’t get any help whatsoever for his mental health, I feel like a fool.

You’re not a fool, it’s just that love makes us blind. You saw some redemption in him despite the red flags, you saw the potential for him if he follow your advice (for a future together, where he will be healthy mentally). Sadly, he didn’t follow through (maybe because he wasn’t ready or doesn’t see the need for him yet : who knows). Also, love includes taking risks (for the future and the relationship/connection we build with our spouse), and that includes that decision you took. You thought it was possible a future with him somehow, and I’m sure you took that decision base on elements/factors that you thought overcame the negatives ones.

Sounds like he’s doing that to be with someone else possibly. Don’t take him back.
