I came across an article talking about discharge

Black discharge to be exact which I recognize as “Old Blood.

The article mentions

Black discharge can also be indicative of accidentally inserting two tampons.

How does one accidentally insert two tampons?

How did you forget ?

How does it fit without you knowing something is it there?!
