5mo will only BF while asleep and reverse cycling - help!

My 4mo will only breastfeed while asleep. He refuses to take a bottle and I am stuck in this constant cycle of getting him to sleep then feeding him - he just protests and fights me at the breast but once asleep (in same position) will feed. He will only feed well overnight (when mostly asleep) and is waking every 2-3hrs from 7pm to 7am for a feed then won't feed well during the day. Any ideas how I can stop this cycle, I am exhausted! He has been checked for tongue ties, reflux etc and is in 98th percentile for weight so supply isn't an issue!!
