Can comfort suckling cause a plugged duct?

My dd is 4w and falls asleep while nursing but she still suckles (pacifies) without swallowing for sometime before she is asleep enough for me to put her down. I know I still have milk in there and after about 20min I feel a sharp needle like pain in my lumpy ducts (which become larger the longer she sleeps), esp the ones in and around the areola. Sometimes she wakes during this time and nurses some more (snacking) and I massage the lumpy areas and milk flows fast bc she gulps and gulps. Curious if this could lead to plugged ducts at some point if it's not emptied. The pain sometimes is great and when the milk is expressed (nursing or pumping) is when I have relief.
