At how many weeks can you take maternity leave and still quilify for Short Term Disability?

I am 30 weeks tomorrow and I feel like I just can't work anymore. I do the full 40 hrs a week and on my feet all day. I'm a dental assistant and I do most of running up and down in the office. There is only 2 of us in the back and the dentist only wants me to assist or do xrays while the other assistant gets to go in the front and help out there. I have very bad sciatica from a previous herniated disc in the past and I get panic attacks (idk why) while working and very dizzy. I talked to my midwife about this but she just says its normal to get these systems while pregnant and I can't technically leave until 36 weeks. I feel so stuck in the position I'm in that I'm getting very depressed about it. I originally planned in working until my last days but didn't expect my back pain to be this bad either.
