Advice quickly!!

So I posted a few days ago about spotting off and on. My doctor won't do anything about it (he's the one I see at labor and delivery too, so going there hasn't helped), just tells me to drink more water. The past few days I've had more spotting, a dull ache in my lower back that feels like back labor (I have three kids at home), period feeling cramps and irregular contractions. I checked myself this morning ( don't freak out, I was taught how to by my nurse so I'm not doing it all the time, this was actually the second this pregnancy, and I know how to do it in a sterile way without causing irritation) and I can easily fit my finger inside without pushing. I know this is all signs of preterm labor, as two of my living kids were born preterm, but with the difficulty I've been having so far with my doctor actually DOING anything has me concerned. Given he's going to be the doctor at labor and delivery, how can I get him to actually help when he hasn't to date? I should also add I'm extremely high risk and get ultrasounds every two weeks for cervical length due to an incompetent cervix and get weekly injections to stop early labor (missed yesterday because I have a seizure disorder and won't risk driving when I'm precursoring to have one).
