Please need advice

So I just found out that my mothers health is getting even worse. Her sister died from being a diabetic at age 46. My mom is 41 and she had a transplant three years ago and is now finding out she is now in kidney failure with only having 41% kidney function. In just a year she lost 35% of her kidney function. She is also high risk for heart attacks and stoke. Im only 19 and found out I have pcos and my boyfriend will be deployed in less then I year for 9 months. I want to give my mom a grandchild so she can can at least see one of them. I know people can live a long time with kidney failure but I just wanna ask for advice. I really want a baby and I want my mom to at least see one of her grandkids in case she doesn't live very long with it because she is showing signs of having heart issues and her sister died from a heart attack from being a diabetic for so long
