Femara question. HELP!

Hi, all! First time interacting on Glow- but I'm a constant reader of the threads :) 
My husband and I have been TTC for over 2 years. In that time, I've done a variety of medications. Birth control to induce periods, metformin, progesterone, clomid, and Femara. 
We had to take a break from TTC due to the husband leaving for deployment. My doctor recommended that I stop all medications and see if my body would generate an AF on its own. Nope, didn't happen. 
Now, I'm on my second round of Femara.
I took 5 days of Progesterone and then I got my AF 4 days ago. It's my first one in 10 months. It's heavy, and painful.... And my poor husband probably has whiplash from my mood swings! I'm supposed to start Femara on CD 5-9. 
So my question is this... Would CD 1 be the day I started spotting? Or the day I actually had a flow? Also, the last time I tried Femara, my doctor said take it CD 5-9 and then do an OPK on days 12-16. However, I bled for 21 days so the entire thing was thrown off! I asked her what to do if that happened, and she said "it probably won't"...... 🙄🙄🙄🙄 
My doctor always seems to be in a rush when I see her. She's been frustrated with me since we met. She didn't think a 21 year old should seek treatment- I know, I married young. I explained to her that I was diagnosed with PCOS when I was 18 and KNEW it would be a struggle. She dismissed that idea really quick. 
And now here we are... 3 years later. And STILL infertile. 
I'm currently looking for a new doctor.
