Tongue Tie and Shallow Latch

Does anyone else have issues with their baby having a shallow latch?  My LO is 3.5weeks and we have had trouble breast feeding since he was born.  It's been very painful for me and we have to supplement with formula via a lactation aid because he lost a lot of weight after he was born.  Our midwife noticed a tongue tie that we had released at 2 weeks.  At our one week follow up it had reattached so they release it again. His tongue moved more freely now but his latch is still so shallow he is always on my nipple, or has a good latch then slips off while feeding.  We have gone to lactation consultants and also having chiropractic treatments on his neck.  We are trying so hard to make breast feeding work but I'm feeling very defeated.  It's so painful I am dreading each feed!  Does anyone else have a similar problem?
