I'd say it depends on their attachment to the pacifier. My friends doctor suggested she start taking away to paci from her 4month old so it wouldn't be more difficult as she got older. My son only takes his when he feels like it so I don't think I'll have to decide when to take it away.

Definitely 2 years max. I worked at a daycare. A 3.5 year old girl was always given a pacifier by her parents. Her teeth started pointing outwards and her top lip was protruding.
