Short term disability question

How does this work for maternity leave? My job doesn't pay maternity leave at all, nothing. So how would I be able to inquire about receiving short term disability for 6-8 weeks? Would my boss need to contact her insurance company? Need some advice please!

We don't have health insurance either. I don't know what to do.
