Where do you draw the line for certain care and isolation?

I work in the Er. My coworkers are very good about not giving me patients who would have any type of isolation. I occasionally have someone with c-diff but even then they try hard to avoid it. I certainly won't do "rashes", unexplained migraines w/fever (meningitis), TB. Things like that I draw my line.

I have a patient with Herpes and open sores all over and I didn't feel comfortable.

I didn't worry about it or say anything just had whatever patients I was assigned. I was extra careful with my PPE and hand washed way more then usual and carried extra scrubs.

It's a personal decision for you - but ultimately, you should not take patients who have precautions for scabies, nuclear med, TB, or flu (especially as we get into flu season!).
