You don't HAVE to do anything. But it's highly recommended, and I wouldn't want to risk having GD and not knowing. It's really not that bad at all.

I'm 28w today and my midwife at last appointment said she wasn't sure I needed one.  I've got an appointment on weds so I'll ask her again.

Fwiw, I have a normal pregnancy, no risk factors, measuring on target with less than average weight gain. I also had a sugar tolerance test at 14w as a baseline due to my dad having steroid induced diabetes at the end of his life. My result on that was completely normal.

You can decline it. I have for four pregnancies now. I have opted to do an A1C/hemoglobin blood draw instead. It gives a more average look at your long term A1C.

It's recommended but I have heard that some people don't do it. You just need to be very mindful that having gestational diabetes that isn't controlled can result in a big baby. If baby is too big then they may need to do a c-section. I had gestational diabetes and even though I was taking medication baby still had to spend a night in NICU because they had to get her blood sugar stabilized. I did have a vaginal birth and she was 6 lbs when born but I was induced early as well. GD and other issues can put you more at risk for preeclampsia also.

As far as I'm aware you only get it if you're at risk, in the U.K. It's related to your BMI and family health history, I think.

Technically you can refuse any testing, but it's recommended for everyone because you can get it even without personal or family history of issues.  Even women with perfect diets and health outside of GD can have it.  
The test is really not as bad as people make it out to be either.
