How old is this little one? Do you use powder? I discovered we absolutely cannot give any citrus to our 17 month old son...no orange juice, fresh oranges, or canned mandarin oranges. Also we cannot use powder as it has caused a rash every time we have used it.  Also make sure your using fragrance free wipes. Wal-Mart Parent's Choice Fragrance Free wipes are the best.  Watch for any poop diapers and change those immediately.  Last week we he had a few times where he passed gas and "sharted" in his diaper. I didn't realize it and he went around with that in his diaper a bit and it caused a red irritated rash that took me two days to get cleared up.  Also  rinse really good after baths to make sure there is no residual baby wash/soap residue. Hope your little one's bottom gets better soon. 😔 Poor baby.

It looks like it could be a yeast infection. You can try coconut oil. It has anti-fungal properties that will take care of the yeast. And as always call your doc because if it is yeast it spreads like mad!

Try aquaphor, my oldest son had a horrific diaper rash for way too long. Found aquaphor and I have never used anything on him or my second son again

Not sure where your from but every time.my little brother got a bad rash I put bag balm on it and it was cleared up within 2 days. I think tractor supply has it and I know the Walmart down here does.

One of my coworkers said the other day it was all the diapers he was using...they had tried different creams and such for the rash. The only thing that worked was switching brands until he found something that didn't leave a rash...ended up being Pampers something er other.

Not sure if that'll help but I never would have thought of that if he hadn't said anything.

Anti-fungal cream like lotramin or regular maalox. Mix either with diaper cream or dab the maalox on gently. 
Also, try using a damp paper towel and then pat dry with dry paper towel instead of wipes until it's healed. 
Sounds silly but both the lotramin or maalox will help correct the PH balance down there.

Triple paste and domboro solution is a life saver. My daughter has a rash 10x worse and after using those two above was healed in 3 days! I swear by the domboro solution
