breast to bottle but how ?

I want introduce a bottle with my breastmilk to my 4 week old but she eats randomly now sometimes every 3 hours sometimes less and if she's awake then she's cluster feeding or just wants milk constantly.

She empties my breast when she feed so I barely have any extra milk when I go pump after a feeding, even 1-2 hours after so then I just let her nurse again and never end up filling a bottle enough for her next meal.

How do I switch completely from breast to bottle (pumped milk)  with her random feeding schedule ?

You'll need to get a few feedings worth pumped out before you switch over. You can pump after each feeding to get any extra left in there. It might take a few days to get enough for a couple feedings. Once you have some bottles, you should be able to give a bottle when your baby is hungry and get yourself on a pumping schedule. My son is almost 6 months and still doesn't have a set feeding schedule. We just feed him when he's hungry.

I supplemented formula one meal a day for a while so I could pump and get ahead two days worth of milk total in 2oz baggies.. then switched back to almost 100% breast milk when the stash was built.

You have to gradually introduce bottles as they can refuse them but perseverance is key! Once you have decided on the bottles, you obviously sterilise them before each feed. 
in the morning fill as many as you think you need with, let's say, 3 ounces of boiled water and leave it all day. It will stay sterile for 24 hours, although I never leave mine longer than 18. 
When it's time for feeding, just top up the pre made bottle of water with say another 2 ounces recently boiled, never leave longer than 30 mins in a kettle, and not only will it be the perfect temp, it will be on demand!
I do it now with my 9 month old. He only has two bottles a day now at nighttime, but I Make 4 ounces water before bed and top it up with 3 more ounces and powder when it's feeding time.
