how do you stay motivated?

Does anyone else get really motivated to change something about their life, and get bored after a few weeks? I've tried starting so many things (e.g. Learning an instrument or a language, exercising, eating healthier, meditating, writing, reading more, etc) and I'll start small and all that and go really strong and then after a few weeks just lose interest and stop! I know this is normal but it's so frustrating that I'm so freaking lazy. 
Are you like this too, and did you have any tips that finally helped you overcome it?
