Medical intervention cascade effect

What do you know about the medical intervention cascade effect when pertaining to birth?
I've been learning a lot about how each medical intervention (MI) can cause another, bigger intervention to need to happen.
For example, when pitocin is used to jump-start labor, the uterus can clamp down too hard making contractions too painful, too strong and much too long for both mother and Baby to endure. Too strong of contractions can cause decreased blood flow and oxygen to Baby causing baby's heartbeat to slow down. This may be alarming and be enough to require an emergency C-section. 
This is just one of many examples of the cascade effect. 
(Other interventions include: epidural, breaking of waters, stripping membranes, use of forceps/vacuum, etc.)
