4 month old help?!

My daughter is 4 months old, she's just getting over a bad fever and today the top of her vagina has creases and inside the creases it's red and raw. I would think maybe her diaper is rubbing her raw, but her diaper doesn't hit right there. Idk what it is. Any ideas? 
She screams bloody murder if it's touched and even when I'm just opening her diaper she screams as if she knows that's where the pain is. It's so weird. Any help would be appreciated!

Yeah every time you change her, put diaper rash cream on her and then rub with a wipe in small circles gently the next time you change her to clean the old cream off, reapply the cream thick and on every part of the redness. Change her diapers often and make sure they aren't tight and fit her well.  They may have got too small for her so you may need to upsize her diaper or that brand might not hold the pee well. 
Give her reassurance while you clean her and try to make it as easy for her as possible.

Baking soda bath, it will help with the inflammation. She may have urine irritation so it's burning. If possible keep a diaper off of her after a bath to air dry.

If she doesn't let you put cream on her try just plain corn starch! I have been using it with all of my children when they would get a rash and my mom and grand parents used it too!

Instead of diaper cream, try coconut oil. Not only will it soothe and heal, but it'll also add a "protective layer" and prevent it from getting worse

I am using Desitin and just let her play naked for an hour. 
Gonna keep doing that often. Thank you all for your ideas!! I didn't realize you could get diaper rash right there!

Also I know the diapers aren't too small cause she just moved up to this size a week ago. 😊

When my son has it in the creases like that, I apply a yeast cream with 1% clotrimazole (like athletes foot cream).  You will know by the next day if it was yeast because it will look much better.

Leave her diaper off for an hour before and after bath if possible and apply diaper cream make sure when changing her diaper that she's dry before applying cream or applying new diaper
