Really bad skin, advice please!

Every pore on my face is filled, I have blackheads all over my chest and it looks like some on my arms, pimples on my neck, shoulders, chest, and some on my face. And eczema on my hands, around my mouth and in between my eyebrows...what can I do to fix this? 😭
