Where do you guys pump when you're outside?

Like when you go shopping at the mall or to a public place? I usually pump every 3-4 hours so if I'm outside for longer than that, I will find a cafe and pump using a nursing cover. I feel a bit shy doing that sometimes, but I can't think of a better solution.

For exclusively breastfeeding mothers, don't you feel the need to pump when your baby is sleeping or isn't available to eat? Do you have to be with your baby 24/7? I honestly don't understand how is it possible to not pump at all.
